Application Requirement
Emergency Travel Certificate
- Sworn affidavit before either a Magistrate or High Court inMalaysiaabout the loss of passport.
- Clear Photocopy of data page of international passport.
- Police report about the loss of passport.
- Possession of a Nigerian driver’s license or a letter of recommendation/acceptance of liability from a Nigerian who is registered in theMissionwith valid resident permit inMalaysia. The recommendation should include known resident address inNigeria, and accompanied with two passport photograph measuring 52mm x 52mm, a copy of his or her standard passport.
- A confirmed one-way ticket.
- A police certificate indicating that applicant is not under police investigation of pending court cases(s).
- Two passport-sized photographs measuring 52mm x 52mm of applicant with white background.
- Mandatory payment of Rm500.00 emergency travel certificate.
Marriage Declaration
- Letter from Nigerian groom stating his single status (bachelor) addressed to the Nigeria High Commission.
- Letter of application to the High Commission for declaration of marriage by both parties to the marriage.
- Letter of acknowledgement from bride-to-be stating that she is consenting to the marriage. Attach photocopy of passport or I/C.
- Photocopy of first four passport pages of Nigerian groom.
- Two passport sized photograph measuring 52mm x 52mm of applicant with white background from both parties.
- Letter confirming bachelorhood from local government through village head, addressed to the High Commission.
Endorsement of Documents
- Covering letter from the company requesting for endorsement of their documents addressed to the High Commission.
- All relevant documents/certificates to be endorsed for export purpose should first be authenticated by the Ministries of Foreign Affairs and Health before submission to the High Commission.
- A certificate of analysis/composition of product being exported should be authenticated by the Controller of Drugs of the Ministry of Health, along with the documents for endorsement.
Guidelines for the Issuance of E-Passport
1. General Procedure for Passport Application
- Go to www.immigration.gov.ng
- Click on the Passport Application
- Select the type of Passport you want
- Click start Application Button to start the process
- Fill the Passport Application, check the ‘I AGREE’ check box then click the ‘SUBMIT APPLICATION’ button
- Print Acknowledgement slip
- Click ‘PROCEED’ to print out relevant documents including the Guarantor’s form where applicable.
- Online fee USD65
General Documents required for interview:
- Standard Passport
- Birth certificate/age declaration
- recent color passport photographs
- Photocopy of data page from the applicant
- Guarantor’s form sworn to before commissioner of Oaths/Magistrate/High Court Judge
- Parent’s letter of consent for minors under 16 years.
- Letter of consent from our Nigeria Mission(Those outside Malaysia)
- Marriage certificate where applicable for married women
- Police report in case of lost passport
- Application letter (for lost passport only)
- Administrative charges RM150.00
- Penalty for lost passport USD250
Note:
- Acknowledgement slip and payment receipt plus 2 recent photographs are applicable in all cases
- Print out copy of duly completed application form (2 copies)
- Take printed and signed application forms to Passport office/ Embassy/ High Commission for further processing
- A guarantor must attach the following documents:
i. Photocopy of Data page of Nigerian Standard Passport
ii. Driving license or National Identity Card or school Identity Card and Visa page
iii. One recent color passport photographs
